In the main storyline, in Vault 112, you will go into a virtual reality controlled by Dr. Braun. He (or Betty as he calls himself) tells you to get the mask and kill everyone. This mask can be found in the actual world in Point Lookout. From where you arrive, go north to the abandoned hotel in the town and enter room 1D. You will find a lot of blood and totem poles. Go into the bathroom. In the bath is a skeleton clutching what appears to be a clown mask with a bobblehat. Pick it up. It is the Pint-Sized Slasher Mask. It has 1DR and +5 Melee.
