On the title screen, let all three demos cycle through then start a new game or continue a saved game. The background music won't play. To disable this, you can either lose a life, fight the boss of the level, or reset the game. Note that when the demos are cycling by, don't skip them. If you do, the glitch won't work.
Hard difficulty
Successfully complete the game under the normal difficulty setting.
